What is Post-Acute Care (PAC)
Considered an “after-care” these are services you generally receive “after” or instead of a stay in a hospital. They include, but are not limited to rehabilitation centers, palliative services, and other care requiring skilled, highly trained nursing staff and care agents.
What is Palliative Care?
Palliative care is specialized medial care for people living with the most serious illnesses and chronic conditions. Since palliative care may begin at the time of diagnosis and through treatment, even for terminally ill, it is an intense level of care. This is not hospice care.
